I hereby consent and agree that OMRON Healthcare, Inc., its agents, and others working under its authority, have the right to take or use photographs and/or videos containing my image/likeness (and/or my property). I understand these images may be used for promotional, news, research and/or educational purposes worldwide, including online, now or hereafter known, and for any purpose whatsoever.

I hereby release all rights to exhibit this work in print and electronic form publicly or privately. I waive any rights, claims or interest I may have to control the use of my identity or likeness in the photographs and/or videos and agree that any uses described herein may be made without compensation or additional consideration of me.

I agree to not sue and to release, indemnify and hold harmless OMRON Healthcare, Inc., its affiliates, officers, directors, volunteers and employees and all participating and sponsoring businesses and organizations and their agents and employees, from any and all liability, claims, demands, and causes of action whatsoever, arising out of my participation.

I agree that any personal data collected and processed as part of this agreement, including but not limited to my name, likeness, and contact information, will be handled in accordance with the OMRON Healthcare, Inc. Privacy Policy, which can be found at https://omronhealthcare.com/privacy/. By signing this agreement, I consent to the use of the personal data for the purposes outlined in this agreement, including sharing with necessary third parties, in compliance with applicable data protection laws.

I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
